At Work Assessment A: Thru weeks 13-26 you must gather sufficient evidence of the support you are providing to complete a DMI. This 'At Work Assessment' will enable you to use natural situations at work as evidence collection opportunities.

DMI At Work Assessments Client File Summary: Thru weeks 13-26 you must gather sufficient evidence of the support you are providing to complete a DMI. This 'At Work Assessment' will enable you to use natural situations at work as evidence collection opportunities.
DMI Client self-Assessment: Thru weeks 13-26 you must gather sufficient evidence of the support you are providing to complete a DMI. Assisting your client to self-assess is a valuable component of evidence collection. You must be present when your client completes this form to help your client understand terms and meanings.
DMI Draft & Record of Evidence: The DMI is completed once a client has been employed for a min of 26 wks (208 hrs capped, 260 hrs uncapped)
DMI Client Self Assessment: This form contains questions that relate to a selection of DMI Domains and Items, and the form is divided into work related themes or categories.
